This form is secondary to F0014_A, Public Internet Address Request Form (Request for Serve Space Assignment) or F0034_A, Request for Static Protect. It will not be handled unless a copy of F00x4_A(F0014_A or F0034_A) has already been recieved or is already on file with Network Engineering. Fill out this form online and submit. Assignments will not be made without a signed F00x4_A. Fax signed forms to Network Engineering and Technology at 703-993-3505. All areas in bold italics print are required. This form can also be used to request an alias or entry for a domain names where the server is hosted at an off campus facility. In this case a F0014_A or F0034_A form is not required. See the examples page for instructions on how to fill out the form properly in this case.
This form is to be used if the machine you are registering has multiple domain names, interfaces, IP's or if the machine has a change in MAC Address due to equipment replacement, or changing IP, or Adding a CNAME to an existing machine. In the first row of the table below enter the MAC Address and DNS Name used on F00x4_A to register the server. In subsequent rows enter the Type of Request (New, Change, Remove) in column 1, the type of change to be made (IP, Alias, Interface) in column 2, the new MAC or IP to be changed in column 3, and the DNS Name to be added, changed or removed in column 4.
