To remain compliant with the Payment Card Industry's [PCI] requirements, all groups wishing to have access to credit card processing on any campus, must register their systems with Network Engineering and Technology [NET]. This is so that a properly designed IP Address assignment can be made for the specific use of Point Of Sale [POS] system. This system must be maintained strictly by the user and NET. All POS IP Addresses will be scanned per the PCI requirements that GMU must follow. If you are registering multiple units, please use F0040_A, POS Network Assignment Request and F0040_B, POS System Information Spreadsheet.

Fill out this form online then print the form prior to submitting. Submitting the completed form will generate a work ticket, however assignments will not be made without the signed form.
