Instructions - PLEASE READ - Updated June 20, 2008

To remain compliant with the Payment Card Industry's [PCI] requirements, all groups wishing to have access to credit card processing on any campus, must register their systems with Network Engineering and Technology [NET]. This is so that a properly designed network segment can be allotted for the specific use of Point Of Sale [POS] systems. This segment must be maintained strictly by the user and NET. All POS networks will be scanned per the PCI requirements that GMU must follow.

Fill out this form online then print the form prior to submitting. Submitting the completed form will generate a work ticket, however assignments will not be made without the signed forms and a completed F0040_B POS System Information. The spreadsheet (F0040_B POS System Information) must be downloaded and completed, an example of the completed form can be found on the Examples tab in the spreadsheet. Once this form is submitted, you will be contacted as to where to send the spreadsheet. If you wish to only register a single POS unit, please complete F0041_A, POS IP Assignment Request.
