Purpose

This form is to be used to request access for staff to use the VPN connection supplied by Network Engineering and Technology. For vendor use complete F0020_A, Vendor VPN Request. If this VPN request is to be used for Teleworking, please turn in forms to Human Resources prior to filling out this form. Information on Teleworking can be found on the University's Telework Policy page: http://www.gmu.edu/facstaff/policy/newpolicy/2202.html .
